When sourced near the scalp, hair offers nearly a 3-month timeframe for detecting drugs and alcohol. Hair collection is straightforward, relatively hard to tamper with, and easy to transport.

A 1.5-inch segment comprising roughly 200 strands (approximately the width of a #2 pencil) close to the scalp yields about 100mg of hair, which is optimal for both screening and confirmation. For EtG, add-ons, or tests over 10 panels, a specimen weighing 150mg is advised. We suggest measuring the sample on a jeweler’s scale. If scalp hair isn't accessible, the same quantity of body hair can be gathered. When mentioning head hair, it specifically refers to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory workflow for processing a drug test result encompasses four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory's system. This step involves confirming that the sample was properly sealed and transported, assigning it a unique LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening serves as a preliminary assessment for drugs of abuse. While cost-effective for negating drug usage in most samples, a positive screen must be affirmed to be legally viable. Samples with presumptive positive results in Screening necessitate further verification.

If a sample tests presumptively positive during Screening, additional hair is taken from the original sample for Extraction. During Extraction, drugs are isolated from hair at much lower concentrations than other methodologies (such as urine or oral fluid), which is why hair drug screening is the most complex methodology.

Confirmation of any positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if required. The complete laboratory process, from Accessioning to Confirmation, is conducted following both the CAP (College of American Pathologists) Hair classification and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
