What it is, How it works

When sourced near the scalp, hair can indicate drug and alcohol usage within a roughly three-month period. Hair collection is straightforward, moderately resistant to tampering, and easily transportable.

A hair sample measuring 1.5 inches in length and consisting of about 200 strands (comparable to the thickness of a #2 pencil) from the scalp yields 100mg, which is the ideal amount for initial tests and confirmations. For tests involving EtG add-ons or exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for accurate measurement. If scalp hair isn't an option, a similar quantity of body hair may be utilized. In discussing head hair, we specifically mean scalp hair. Body hair encompasses all other types (e.g., facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result follows four essential steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the stage where the sample is initially integrated into the laboratory system. This process ensures proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing data entry that an electronic chain of custody system didn’t provide.

Screening is a preliminary assessment to detect drugs of abuse. While this step is cost-effective for excluding drug use in most samples, any positive finding in Screening requires confirmation for validity in legal settings. Samples testing potentially positive in Screening must undergo further verification.

Should a sample yield a potential positive result during Screening, additional hair is extracted from the original sample for the Extraction step. At this phase, drugs are isolated from hair at significantly lower concentrations compared to methods like urine or saliva testing, rendering hair drug screening the most challenging to conduct.

Confirmation for any positive results from Screening is executed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Any samples with potential positives are cleansed prior to Confirmation as necessary. The entire process, from Accessioning to Confirmation, adheres to both the CAP (College of American Pathologists) Hair specifications and ISO / IEC 17025 accreditation standards.

The SoHT meeting in Verona emphasized the significance of rigorous interpretation of analytical results and the need for standardized protocols. Manuscripts in this section present advancements in forensic toxicology, and have explored the challenges posed by emerging designer drugs and strategies to combat their proliferation. The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
