What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol consumption. Hair offers an extended timeline of alcohol and drug usage by capturing biomarkers within the hair strand's fibers as it grows. When obtained from near the scalp, hair can provide about a 3-month detection period for alcohol and drugs. Collecting hair is straightforward, relatively hard to tamper with, and convenient to transport.

A sample measuring 1.5 inches with approximately 200 hairs (roughly the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, the optimal amount for both screening and confirmation. For tests involving EtG, additional analyses, and/or more than 10 panels, 150mg of the specimen is advised. It's suggested to weigh the sample on a jeweler's scale. If scalp hair is not available, a similar quantity of body hair can be used. References to head hair pertain solely to scalp hair. Body hair includes all other types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initially processing a sample into the laboratory's system. This step includes confirming that the sample was properly sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any missing data entry not covered by an electronic chain of custody system.

Screening involves a preliminary examination for drugs of abuse. While Screening is a cost-efficient approach to eliminate drug usage on the majority of samples, a positive screen must be confirmed for it to be legally valid in court. Any samples presumptively positive in Screening require subsequent confirmation.

If a sample shows presumptive positivity during Screening, additional hair is extracted from the original sample and prepared for Extraction. In this phase, drugs are removed from hair at a significantly lower concentration compared to other techniques (e.g., urine or oral fluid), making hair drug screening a challenging methodology.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed prior to confirmation as necessary. The complete laboratory process from Accessioning to Confirmation is conducted in accordance with both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
