If scalp hair is lacking, a similar quantity of body hair can suffice. When mentioning head hair, it specifically refers to scalp hair. Body hair encompasses all other categories (facial, axillary, and so on).

Process Overview

The laboratory handling of a drug test result encompasses four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial intake of a sample into a lab’s system. This step includes confirming the sample was correctly sealed and dispatched, assigning it a random LAN (Laboratory Accessioning Number), and completing any further required entries not facilitated by an electronic chain of custody.

Screening offers a preliminary rapid assessment of potential substance abuse. While Screening is a cost-efficient method for dismissing drug use in the bulk of samples, a positive result requires confirmation to be legally valid. Samples showing preliminary positive results during Screening necessitate further Confirmation.

Here, substances are isolated from hair at significantly reduced concentrations compared to other methods (such as urine or oral fluids), rendering hair drug testing as one of the more complex methodologies to execute.

Verification of any Screening positive outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All tentative positive samples undergo washing before the Confirmation process as warranted. The entire lab cycle, from Accessioning through Confirmation, is scrutinized under CAP (College of American Pathologists) Hair protocols and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
