Collecting hair near the scalp can yield insights over a roughly three-month period for both alcohol and drugs. The sample collection process is straightforward, relatively resistant to tampering, and easy to transport.

An ideal sample consists of a 1.5-inch section of around 200 hairs (approximately the width of a #2 pencil) taken closest to the scalp, providing 100mg for optimal screening and validation. For EtG, additional tests, or panels exceeding 10, 150mg is advisable. We suggest using a jeweler’s scale for accurate measurement. If scalp hair is unobtainable, a comparable amount of body hair may suffice. "Head hair" refers specifically to scalp hair, whereas "body hair" includes all other hair types (e.g., facial, underarm).

Process Overview

The lab procedure for analyzing a drug test comprises four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is entered into the laboratory's system. This step ensures the sample was appropriately sealed and delivered, assigns a unique LAN (Laboratory Accessioning Number), and addresses any missing data entry not captured by an electronic chain of custody.

Screening involves a preliminary evaluation for substance misuse. Though Screening is cost-efficient for excluding drug use in most instances, any positive findings require confirmation for legal admissibility. Samples with preliminary positive results in Screening must undergo further confirmation.

For samples with potential positive results from Screening, additional hair is retrieved from the original sample for the Extraction phase. This step involves isolating drugs from hair at significantly lower concentrations than other methods (e.g., urine or saliva), making hair drug testing the most challenging to conduct.

Positive results from the screening are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, presumptive-positive samples are cleaned as necessary. The complete lab process, from Accessioning to Confirmation, is conducted in adherence to both the CAP (College of American Pathologists) Hair standards and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
