A hair sample measuring 1.5 inches, consisting of roughly 200 strands (comparable to the diameter of a #2 pencil) closest to the scalp yields about 100mg of hair, the optimal amount for initial testing and subsequent confirmation. For EtG enhancements and/or tests involving more than 10 panels, a specimen of 150mg is advised. It is suggested to weigh the specimen with a jeweler’s scale. If scalp hair is unavailable, a similar quantity of body hair can be used. In terms of head hair, this refers solely to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary phases in the laboratory processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the foundational processing of a sample within a laboratory’s system. This includes confirming the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry absent from an electronic chain of custody system.

Screening is a preliminary assessment intended to identify potential drugs of abuse. While Screening offers a cost-efficient method of excluding drug usage in the majority of samples, a positive screening result requires confirmation to be deemed court admissible. Any samples showing presumptive positives in Screening necessitate further confirmation.

If a sample is deemed presumptively positive at the Screening stage, additional hair will be drawn from the original specimen for Extraction. Within this phase, drugs are extracted from hair at significantly lower concentrations than in other approaches (e.g., urine or oral fluid), highlighting why hair drug testing is one of the most challenging methodologies to execute.

Confirmation of any positive screening outcomes involves GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
