What it is, How it works

When collected near the scalp, hair can provide a detection window of up to roughly 3 months for alcohol and other drugs. The process of collecting hair is straightforward, somewhat resistant to tampering, and convenient to transport.

A 1.5-inch sample consisting of about 200 hair strands (approximately the size of a #2 pencil) taken nearest to the scalp yields 100mg of hair, which is the ideal sample amount for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, a specimen weight of 150mg is advisable. We recommend using a jeweler’s scale for weighing the specimen. If scalp hair is not available, an equivalent amount of body hair may be collected. The term "head hair" refers exclusively to scalp hair, while "body hair" encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is integrated into a laboratory’s system. This process includes verifying that the sample was sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not covered by an electronic chain of custody system.

Screening involves a preliminary quick evaluation for drugs of abuse. While Screening is a cost-efficient method for dismissing drug usage on most samples, a positive screen requires confirmation to be valid in court. Any samples that test presumptively positive in Screening necessitate a secondary confirmation.

Should a sample test presumptively positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. During this stage, drugs are extracted from hair at concentrations significantly lower than those in other testing methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies to conduct.

Confirmation of any positive screening outcome is done using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples are washed before confirmation as needed. The complete laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
