However, urine tests remain superior for detecting recent drug use within five days.

What it is, How it works

Utilizing hair testing has emerged as an effective method for uncovering the use of drugs and alcohol. Hair retains a prolonged account of alcohol and drug consumption by embedding biomarkers in the hair strand as it grows. When hair is gathered near the scalp, it can offer a detection period extending up to three months for alcohol and drugs. Hair collection is straightforward, somewhat challenging to tamper with, and effortless to transport.

Gathering a 1.5-inch segment consisting of roughly 200 strands of hair (equivalent to a #2 pencil's diameter) closest to the scalp will yield 100mg of hair, which is the optimal sample size for testing and verification. For EtG, additional tests, and/or for panels exceeding 10, a specimen weighing 150mg is recommended. It's advisable to measure the specimen with a jeweler’s scale. If scalp hair cannot be obtained, a similar amount of body hair is acceptable. By head hair, we mean scalp hair. Body hair denotes all other hair types (such as facial or underarm hair).

Overview of the Process

The laboratory processing of drug test outcomes encompasses four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the preliminary processing of a sample within a laboratory’s framework. This step involves confirming the sample's proper sealing and shipping, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any remaining data entry not supplied by an electronic custody chain system.

Screening is a quick initial assessment for substance abuse detection. Though Screening is a cost-efficient approach to dismiss drug use in most cases, a positive result requires confirmation for legal acceptance. Any samples testing positive during Screening must undergo a secondary validation test.

Should a sample test presumptively positive at the Screening phase, additional hair is extracted from the initial sample for Extraction preparation. In this phase, drugs are extracted from hair at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most complex method to undertake.

Positive screening results undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. All samples testing presumptively positive are cleansed before Confirmation as necessary. The entire laboratory procedure, from Accessioning to Confirmation, adheres to CAP (College of American Pathologists) Hair designation and accreditation under ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
