A measurement of 1.5 inches from the hair nearest to the scalp, comprising roughly 200 strands (similar in size to a #2 pencil), will yield 100mg, which is the optimal amount for testing and verification. For EtG, additional tests, and/or screenings beyond 10 panels, 150mg of the sample is advised. Utilizing a jeweler’s scale for weighing the specimen is recommended. If scalp hair is absent, an equivalent quantity of body hair can be used. References to head hair pertain solely to scalp hair. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

There are four primary phases in the lab procedure for processing a drug test outcome: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary handling of a specimen within a lab’s infrastructure. It involves confirming that the sample is properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry omitted by an electronic chain of custody system.

Screening involves an initial swift check for drug abuse. Although Screening is an economical approach to negate drug use for most specimens, any positive screen must be confirmed for court admissibility. Thus, samples that are initially positive in Screening necessitate a secondary confirmation.

If an initial positive is yielded during the Screening process, extra hair is taken from the original specimen for Extraction preparation. At this stage, drugs are extracted from hair at lower concentrations than with other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening as opposed to other methodologies.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS technologies. All samples deemed presumptively positive are cleaned prior to confirmation as required. The comprehensive lab procedure from Accessioning to Confirmation is supervised under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
