What it is, How it works

Hair analysis has gained recognition as an effective method for detecting drug and alcohol consumption. Hair captures a long-term record of alcohol and drug intake by enclosing biomarkers in the strands of the growing hair. When sampled near the scalp, hair can offer a detection window of up to around 90 days for alcohol and drugs. Collecting hair is straightforward, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch cut of about 200 hair strands (comparable to the thickness of a #2 pencil) from closest to the scalp will yield 100mg of hair, which is the preferred sample for examination and verification. For EtG, additional tests, or panels exceeding 10, a specimen weight of 150mg is advisable. Measuring the specimen using a jeweler’s scale is suggested. If scalp hair is unavailable, a commensurate amount of body hair may be obtained. By head hair, we mean scalp hair exclusively. Body hair refers to all other types of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test outcomes consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial handling of a sample into a lab’s records. This process ensures that the sample was correctly sealed and dispatched, assigns a random Laboratory Accessioning Number (LAN), and completes any outstanding data entry that an electronic chain of custody system doesn’t cover.

Screening offers a preliminary rapid analysis for drugs of abuse. Although Screening provides a cost-efficient method to eliminate drug use in most samples, a positive screen must be validated to be legally acceptable. Any samples that exhibit a presumptive positive in Screening demand further confirmation.

Should a sample demonstrate a presumptive positive during Screening, additional hair is collected from the original specimen and readied for Extraction. In this phase, drugs are retrieved from the hair at a concentration much lower than other methods (such as urine or saliva), making hair drug analysis the most challenging method to conduct.

For any positive screening results, the Confirmation is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All samples showing a presumptive positive are cleaned before confirmation as required. The complete lab procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation to ISO / IEC 17025 benchmarks.

Strengths of hair drug testing:

  • Resistance to tampering: Hair drug tests are exceptionally difficult to manipulate, resulting in more precise results.
