Securing a 1.5-inch segment of roughly 200 hair strands (similar in size to a #2 pencil) nearest to the scalp yields 100mg of hair, which is the preferred sample size for screening and affirmation. For EtG, additional tests, and/or analyses beyond 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the specimen is suggested. If scalp hair is not present, an equivalent quantity of body hair can be gathered. In the context of head hair, we mean only the scalp hair. Body hair comprises all other types (facial, axillary, etc.).

Process Overview

The four key phases in the lab handling of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a specimen into a laboratory’s framework. This encompasses confirming the sample was appropriately sealed and dispatched, designating a random LAN (Laboratory Accessioning Number), and concluding any extra data input not supplied by an electronic chain of custody system.

Screening entails a preliminary swift assessment for drugs of abuse. While Screening offers a cost-efficient method to exclude drug consumption in most samples, a positive screen must be verified to stand in court. Any samples showing presumptively positive in Screening necessitate subsequent confirmation.

Should a sample be presumably positive during the Screening stage, more hair is extracted from the initial specimen and made ready for Extraction. In this phase, drugs are withdrawn from hair at a significantly dilute concentration than in other approaches (e.g., urine or oral fluid), making hair drug screening a challenging technique to execute.

Confirmation of any affirmative screen result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
