What it is, How it works

Hair testing has emerged as an effective method for identifying drug and alcohol consumption. Hair offers an extensive history of alcohol and other drugs by embedding biomarkers within the strands as they grow. When collected near the scalp area, hair can reveal a detection timeframe of up to three months for alcohol and drugs. Hair is straightforward to gather, hard to tamper with, and simple to transport.

A 1.5-inch length consisting of around 200 hair strands (comparable to a #2 pencil) closest to the scalp will yield 100mg, the optimal quantity for assessment. For EtG, additional tests, and/or examinations beyond 10 panels, a 150mg sample is advisable. Utilizing a jeweler’s scale to weigh the sample is suggested. If scalp hair is lacking, a comparable quantity of body hair is an alternative. When discussing head hair, it pertains exclusively to scalp hair, while body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

Accessioning, Screening, Extraction, and Confirmation are the four primary stages involved in the lab processing of a drug test result.

The Accessioning stage handles the initial sample processing into the lab’s system. This step confirms that the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data input absent from an electronic chain of custody framework.

Screening provides a primary rapid check for drug abuse. While it is an economical method to exclude drug use in most cases, a positive screen demands confirmation for judicial admissibility. All samples with a presumptive positive in Screening mandate a subsequent confirmation.

When a sample shows presumptive positivity in Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a far lower concentration than with other methods (e.g., urine or oral fluid), making hair drug testing a challenging process.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Every presumptive-positive sample is washed before confirmation, if required. The comprehensive lab process from Accessioning through to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The pitfalls encountered when the exogenous/endogenous nature of a substance is questioned were presented and discussed. The special issue presents also manuscripts that delve into the ethical implications of drug testing in drug facilitated sexual assaults.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
