A sample of 1.5 inches consisting of about 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, the preferred amount for analysis and validation. For EtG, additional tests, or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to measure the specimen. In the absence of scalp hair, a similar amount of body hair can be used. By head hair, we exclusively mean scalp hair. All other types (like facial, underarm, etc.) are classified as body hair.

Process Overview

The laboratory process for drug test results involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the first phase where a sample is integrated into a lab’s system. This phase checks the sample’s proper sealing and shipment, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing data entry if not provided by a digital chain of custody system.

Screening is a preliminary check for drugs of abuse. It is an economical way to exclude drug usage in most samples, but a positive result in Screening must be confirmed for legal use. Samples that are preliminarily positive in Screening require further validation.

Once a sample tests presumptive positive during Screening, additional hair is taken from the initial sample to prepare for Extraction. In this phase, lower concentrations of drugs are extracted from hair compared to other methods (like urine or saliva), which makes hair drug screening the most complex methodology.

Confirmation of any positive screening finding is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as needed. The full lab process from Accessioning to Confirmation complies with both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
