What it is, How it works

Hair captures a prolonged history of alcohol and other substances by embedding biomarkers in the strands of growing hair. When collected near the scalp, hair can provide a detection period of up to approximately three months for alcohol and other substances. Hair is straightforward to gather, somewhat challenging to tamper with, and simple to transport.

Collecting a 1.5-inch section comprising around 200 hair strands (akin to the diameter of a #2 pencil) nearest to the scalp results in 100 mg of hair, which is the optimal sample for initial and confirmatory testing. For EtG, supplementary tests, and/or evaluations beyond 10 panels, a specimen amount of 150 mg is advisable. We suggest verifying the specimen's weight on a jeweler’s scale. If scalp hair is unavailable, a comparable quantity of body hair can be collected. By head hair, we mean only scalp hair, whereas body hair encompasses all other types (including facial, axillary, etc.).

Process Overview

The four essential steps in the laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s system. This step verifies the proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data entry not covered by an electronic chain of custody system.

Screening conducts an initial rapid assessment for drug abuse. Although Screening is an economical method to eliminate drug usage for the majority of samples, a positive screen requires confirmation for court admissibility. Presumptively positive Screening samples necessitate further confirmation.

If a sample is presumptively positive during Screening, additional hair is taken from the original specimen and readied for Extraction. In this process, drugs are extracted from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), making hair drug analysis the most complex methodology.

Confirmation of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are cleansed as necessary. The complete laboratory process from Accessioning to Confirmation is monitored under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
