What it is, How it works

When gathered near the scalp, hair can offer up to a 3-month detection timeframe for alcohol and drugs. Hair is easy to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch section consisting of approximately 200 strands of hair (about the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal amount for screening and confirmation purposes. For tests involving EtG, additional panels, or more than 10 panels, 150mg of the specimen is advisable. We suggest using a jeweler’s scale to measure the specimen. If scalp hair cannot be obtained, an equivalent quantity of body hair can be used. When we mention head hair, it specifically means scalp hair. Body hair covers all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial handling of a sample within a laboratory’s system. This step verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and fills in any missing information not available through an electronic chain of custody system.

Screening conducts an initial rapid assessment for drug presence. While Screening is an economical way to eliminate drug use in most samples, a positive screen must undergo Confirmation to be permissible in legal settings. Any samples that show a presumptive positive during Screening necessitate additional confirmation.

For samples with a presumptive positive in the Screening phase, additional hair is procured from the original sample and prepared for Extraction. During this phase, drugs are extracted from hair at significantly lower concentrations than other testing methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS techniques. Before confirmation, all presumptive positive samples are cleansed as required. The entire laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
