What it is, How it works

Taking a 1.5-inch segment consisting of around 200 hairs (similar in thickness to a #2 pencil) from closest to the scalp yields 100mg, which is the preferred quantity for both screening and confirmation tests. For EtG, additional tests, and/or testing beyond 10 panels, a sample of 150mg is advised. It's best to measure the sample on a jeweler’s scale. If scalp hair isn't an option, a similar mass of body hair can be collected. In terms of head hair, we specifically mean scalp hair. Body hair includes all other hair types (such as facial and underarm hair).

Process Overview

The essential phases in laboratory processing for a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab's system, ensuring the sample was properly sealed and sent, assigning a LAN (Laboratory Accessioning Number), and completing data entry absent from an electronic chain of custody record.

Screening provides an initial rapid assessment for drug presence. While cost-effective for excluding drug use in most samples, a positive screen needs confirmation to be legally valid. Any samples returning a presumptive positive in Screening require confirmation through a follow-up test.

For samples that are presumptively positive in Screening, additional hair is extracted from the original specimen for preparation in the Extraction stage. Here, drugs are isolated from hair at a comparatively lower concentration than in methods like urine or oral fluid, contributing to why hair drug testing is one of the toughest methodologies to execute.

Positive screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS technologies. Before confirmation, all samples with presumptive positives are cleaned as necessary. The entire lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair protocol and accreditation in compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
