What it is, How it works

Hair testing is recognized as an effective means for identifying drug and alcohol consumption. Hair captures a prolonged record of these substances by embedding biomarkers within the fibers of the hair as it grows. When gathered near the scalp, hair can reveal up to a 3-month detection window for alcohol and drugs. It's easy to collect, challenging to tamper with, and convenient to transport.

A sample of 1.5 inches totaling around 200 hair strands (approximately the width of a #2 pencil) nearest the scalp provides 100mg of hair, which is optimal for screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advisable. We suggest using a jeweler’s scale to measure the specimen. If no scalp hair is available, a similar amount of body hair might be used. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample into a lab’s system. This encompasses confirming the sample’s seal and appropriate shipping, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not supported by an electronic chain of custody system.

Screening includes an initial rapid assessment for drug abuse. Although Screening is a cost-efficient method to exclude drug use in most samples, a positive screen must be verified to be acceptable in legal settings. Samples that are preliminarily positive during Screening require a secondary confirmation.

When a sample is preliminarily positive in the Screening stage, additional hair is extracted from the original specimen and processed for Extraction. During this stage, drugs are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is completed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are cleansed prior to confirmation as necessary. The overall lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
