A 1.5-inch segment of around 200 hair strands (akin to the size of a #2 pencil) closest to the scalp yields 100mg of hair, which is the perfect sample for analysis and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the sample is suggested. It is advisable to weigh the sample on a jeweler's scale. If scalp hair is unobtainable, an equivalent quantity of body hair may be gathered. By head hair, we are expressly referring to scalp hair. Body hair denotes all other hair forms (facial, axillary, etc.).

Process Overview

The four key phases in the laboratory processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a laboratory’s database. This process verifies that the sample was correctly sealed and shipped, allocates a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not supplied by an electronic chain of custody system.

Screening constitutes a preliminary swift check for drugs of abuse. Although Screening is a budget-friendly method to discount drug usage on numerous samples, a positive screen necessitates confirmation for legal admissibility. Any samples deemed presumptively positive during Screening mandate a secondary verification.

If a sample is deemed presumptively positive during the Screening stage, additional hair is retrieved from the initial sample for Extraction. In this phase, substances are pulled from hair at significantly lower concentrations than alternative methodologies (e.g., urine or oral fluid), contributing to hair drug screening being the most challenging methodology to administer.

Confirmation for any positive screening result is carried out through GC/MS, GC/MS/MS or LC/MS/MS. All samples identified as presumptive positives undergo a wash prior to confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, is audited under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 norms.

Advantages of hair drug testing:

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
