A hair sample measuring 1.5 inches and consisting of roughly 200 strands (comparable to the diameter of a #2 pencil), ideally weighs 100mg for proper screening and verification. If testing for EtG, additional parameters, or panels exceeding 10, a 150mg sample is advised. It is advisable to verify the specimen’s weight using a jeweler’s scale. In the absence of scalp hair, a proportional amount of body hair may be used. In the context of head hair, we refer specifically to scalp hair. Body hair includes hair from all other areas (such as facial and underarm hair).

Process Overview

The laboratory method handling the result of a drug test encompasses: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of inputting a sample into the lab’s system. This step verifies the sample’s intact seal and appropriate shipping, allocates a random LAN (Laboratory Accessioning Number), and completes any extra data entry absent from an electronic custody system.

Screening provides a preliminary evaluation for substance misuse. While it is an economical method to exclude drug usage across most samples, court-acceptable results from positive screens necessitate a confirming follow-up. Each presumptive positive from the Screening requires supplementary validation.

When Screening yields a presumptive positive, more hair is extracted from the primary sample for the Extraction process. During this step, substances are isolated from hair at concentrations significantly less than in other methods (e.g., urine or saliva), contributing to the complexity of conducting hair drug analysis.

Any positive found during screening must be confirmed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive samples undergo a wash before confirmation if necessary. The comprehensive lab process, from Accessioning to Confirmation, is subject to scrutiny under both the CAP (College of American Pathologists) Hair designation and accredited in compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
