A 1.5-inch segment of approximately 200 hair strands (roughly the diameter of a #2 pencil) near the scalp will provide 100mg, which is the optimal amount for screening and confirmation. For EtG, additional substances, and/or tests exceeding 10 panels, 150mg of hair is suggested. Using a jeweler’s scale to measure the specimen is advisable. If scalp hair is unobtainable, a similar quantity of body hair may be used. By head hair, we specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample within a lab’s system. This step includes ensuring that the sample is properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any data entry not covered by an electronic chain of custody system.

Screening provides a preliminary scan for drugs of abuse. Although Screening offers a cost-efficient way to dismiss drug usage for most samples, any positive result requires confirmation to be legally viable. Presumptive positives from the Screening must undergo a secondary verification.

Should a sample test positive in the Screening phase, additional hair is extracted from the initial batch for the Extraction phase. During this phase, drug extraction occurs from hair at far lower concentration levels compared to other methods (e.g., urine or oral fluid), making hair drug testing one of the most challenging methodologies to execute.

Positive screening results are confirmed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before confirmation if needed. The entire process from Accessioning to Confirmation is thoroughly checked under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
