A 1.5-inch portion comprising about 200 hair strands (approximately the diameter of a #2 pencil) nearest the scalp yields 100mg, the ideal dosage for both screening and verification. For EtG, supplementary tests, and/or tests involving more than ten panels, a specimen of 150mg is suggested. Using a jeweler’s scale is recommended for measuring the specimen. If scalp hair is absent, an equivalent amount of body hair can be used. When we mention head hair, we specifically mean scalp hair. By "body hair," we mean all other forms of hair (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the beginning phase where a sample is initially processed into the lab's system. This step includes confirming that the sample was properly sealed and dispatched, allocating a random LAN (Laboratory Accessioning Number), and completing any necessary data entry absent from an electronic chain of custody system.

Screening is an initial rapid assessment for drug abuse. While Screening acts as a cost-effective measure to exclude drug usage across most samples, a positive outcome in the screening stage requires confirmation for legal admissibility. Samples with preliminary positive results in Screening need a secondary verification.

Should a sample show positive results during the Screening stage, additional hair is taken from the initial specimen for the Extraction process. In this step, drugs are extracted from hair at notably lower concentrations than in other techniques (such as urine or oral fluid), highlighting the complexity of hair drug testing.

The Confirmation phase for any positive Screening results is executed using techniques such as GC/MS, GC/MS/MS, or LC/MS/MS. All samples with preliminary positive results are cleansed before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is assessed under the CAP (College of American Pathologists) Hair designation and adheres to ISO / IEC 17025 standards.
