A sample measuring 1.5 inches of roughly 200 hair strands (similar to a #2 pencil in size) from closest to the scalp will yield 100mg of hair, perfect for screening and confirming. For EtG, supplements, and/or tests exceeding 10 panels, a 150mg specimen is suggested. It's wise to weigh the specimen on a jeweler’s scale. In cases where scalp hair isn't available, an equal quantity of body hair can be obtained. References to head hair pertain strictly to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of entering a sample into a lab’s system initially. This task includes verifying the sample's proper sealing and shipment, creating a random LAN (Laboratory Accessioning Number), and finalizing any extra data entry that the electronic chain of custody system doesn't cover.

Screening entails a quick preliminary drug abuse check. Although Screening is a cost-efficient technique to exclude drug use for most samples, a positive screen must be verified to be court-admissible. Presumptive positive samples in the Screening phase necessitate further confirmation.

During the Screening phase, if a sample tests presumptively positive, more hair is extracted from the original specimen and readied for Extraction. In this step, drugs are drawn out from hair at significantly lower concentrations than those in alternative methodologies (e.g., urine or oral fluid), accentuating the complexity of hair drug screening.

Positive screening results undergo Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumptively positive are washed before confirmation if necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
