What it is, How it works

Analyzing hair has become a valuable method for identifying drug and alcohol usage. Hair captures markers in its growing strands, providing a prolonged history of substance intake. Collected near the scalp, hair offers a detection period of roughly three months for alcohol and other drugs. It's straightforward to collect, hard to tamper with, and convenient to transport.

A 1.5-inch segment from around 200 hair strands (approximately the width of a #2 pencil) near the scalp yields 100mg of hair, which is the optimal sample for both screening and confirmation tests. For EtG, additional substances, and/or tests exceeding 10 panels, 150mg of the specimen is suggested. Using a jeweler's scale to weigh the specimen is ideal. If scalp hair is absent, an equivalent amount of body hair can be used. "Head hair" refers strictly to scalp hair, while "body hair" encompasses all other types (like facial, axillary, etc.).

Process Overview

The drug test result processing in laboratories comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial entry of a sample into the laboratory’s system. It includes confirming the sample's proper sealing and shipping, generating a unique LAN (Laboratory Accessioning Number), and filling in any extra data missing from the electronic chain of custody system.

Screening acts as an initial rapid assessment for drugs of abuse. While cost-effective for eliminating drug use in the majority of samples, a positive screen must be confirmed for court admissibility. Any samples tentatively positive during Screening require subsequent confirmation.

Should a sample seem positive in Screening, more hair is extracted from the initial collection and prepped for Extraction. This stage involves extracting drugs from hair at far lower concentrations compared to other techniques (e.g., urine or oral fluid), which is why hair drug testing proves to be particularly challenging.

Confirmation of any positive screen is carried out using GC/MS, GC/MS/MS, or LC/MS/MS methods. All initial positive samples are washed before confirmation as necessary. From Accessioning to Confirmation, the full laboratory process is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
