What it is, How it works

Hair captures a prolonged history of alcohol and other substances by sequestering biomarkers within the fibers of the hair as it grows. When collected near the scalp, hair can offer a detection window of up to three months for alcohol and other substances. This method is easy to collect, relatively resistant to tampering, and straightforward to transport.

A sample measuring 1.5 inches of about 200 hair strands (roughly the size of a #2 pencil) taken closest to the scalp will yield 100mg of hair, which is the optimal sample amount for preliminary and confirmatory testing. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We recommend measuring the specimen on a jeweler’s scale. If no scalp hair is available, a comparable amount of body hair may be used. When mentioning head hair, we refer specifically to scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four main phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a lab's system. This includes confirming that the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and finalizing any remaining data entry not covered by an electronic chain of custody system.

Screening involves a preliminary rapid check for substances of abuse. While Screening is a budget-friendly method to exclude drug use for most samples, a positive screen must be confirmed to stand up in court. Samples that are presumptively positive in Screening require additional confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the initial specimen and readied for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than in alternative methodologies (such as urine or oral fluid), highlighting why hair drug screening is among the most challenging methodologies to execute.

Confirmation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed prior to confirmation as needed. The entire process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
