What it is, How it works

Hair analysis is recognized as an effective means for identifying drug and alcohol consumption. Hair retains a prolonged history of these substances by capturing biomarkers within the strands as they grow. When sourced near the scalp, hair provides an approximately 3-month period of monitoring for drugs and alcohol. Collecting hair is uncomplicated, not easily tampered with, and convenient to transport.

A 1.5-inch segment of roughly 200 hair strands, similar in size to a #2 pencil, located nearest to the scalp will yield about 100mg of hair, which is ideal for both screening and confirmation. For EtG, additional analysis, and/or when testing exceeds 10 panels, a 150mg sample is advised. Utilizing a jeweler’s scale is recommended for weighing the sample. If head hair cannot be sourced, an equivalent volume of body hair is an acceptable substitute. "Head hair" specifically refers to scalp hair exclusively, whereas "body hair" encompasses all other types (such as facial and axillary hair).

Process Overview

The laboratory process of drug testing encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a sample into a laboratory system. This step ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and completes supplementary data entry absent from an electronic chain of custody system.

Screening is a preliminary assessment for drug presence. While an economical method for excluding drug use in most samples, a positive screening necessitates confirmation to be admissible in legal settings. Samples flagged as presumptively positive in Screening require secondary Confirmation.

If Screening indicates a presumptive positive, additional hair from the initial sample is utilized for Extraction. During Extraction, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine or saliva), underscoring hair drug screening's complexity.

Confirmation of any positive from the Screening stage is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed before Confirmation as necessary. The complete lab process—from Accessioning to Confirmation—is examined under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
