What it is, How it works

Hair captures a prolonged record of alcohol and drug use by embedding biomarkers within the strands as they grow. When taken close to the scalp, hair can reveal a detection period of up to around 3 months for substances like alcohol and drugs. Collecting hair is straightforward, a bit challenging to tamper with, and easy to transport.

A 1.5-inch collection of around 200 hair strands (roughly comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and verification. For EtG, additional testing, and/or panels exceeding 10, it's advised to use 150mg of the hair sample. Using a jeweler's scale is suggested for weighing the hair. If scalp hair cannot be collected, an equivalent body hair sample may be used. The term 'head hair' specifically pertains to scalp hair alone. 'Body hair' includes all other types of hair (such as facial, axillary, etc.).

Process Overview

The primary tasks in laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary processing of a sample within a lab's framework. This step ensures the sample was properly sealed and dispatched, assigns an arbitrary LAN (Laboratory Accessioning Number), and concludes any supplemental data input not managed by an electronic chain of custody.

Screening provides an initial rapid assessment for drug presence. While being a budget-friendly approach to dismiss drug use across most samples, a positive screening requires confirmation to stand in court. Any samples that appear positive presumptively during Screening need further verification.

For samples presumed positive at the Screening stage, additional hair is extracted from the original sample to prepare for Extraction. In this phase, drugs are retrieved from hair at a comparatively lower concentration than in other techniques (e.g., urine or oral fluid), making hair drug testing the most challenging of methods to execute.

Confirmation of any positive result from screening occurs via GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumed positive samples may be cleansed as necessary. The entire lab procedure from Accessioning to Confirmation is governed by both the CAP (College of American Pathologists) Hair guidelines and the ISO/IEC 17025 accreditation standards.
