For EtG, supplemental additions, and/or examinations exceeding 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair is not accessible, a similar quantity of body hair can be obtained. When mentioning head hair, we specifically refer to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The four pivotal stages in the laboratory evaluation of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial entry of a specimen into a laboratory system. It encompasses confirming that the specimen was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening comprises an initial rapid assessment for substance abuse. Though Screening is a budget-friendly approach to eliminating drug use in most specimens, a positive screen necessitates confirmation for court admissibility. Samples that are presumptively positive in Screening mandate a follow-up confirmation.

Should a specimen show presumptive positivity during Screening, additional hair is taken from the original sample and prepared for Extraction. In this phase, substances are extracted from hair at significantly lower concentrations than in other techniques (e.g., urine or oral fluid), making hair drug testing one of the most challenging methodologies to execute.

Confirmation of any positive screen result is carried out via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive specimens are cleansed before Confirmation as needed. The entire laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
