When collected near the scalp, hair presents a detection period of roughly up to 3 months for alcohol and other drugs. It is easy to collect, tough to tamper with, and simple to transport.

A 1.5-inch portion of approximately 200 strands (comparable to a #2 pencil's thickness) nearest to the scalp provides 100mg of hair, which is the optimum quantity for analysis and verification. For EtG, additional tests, and tests exceeding 10 panels, 150mg of the specimen is advisable. We suggest using a jeweler’s scale to weigh the sample. Should scalp hair be unavailable, an equivalent volume of body hair can be utilized. "Head hair" specifically refers to scalp hair, while "body hair" covers all other types (e.g., facial, axillary).

Process Overview

The laboratory processing of a drug test follows four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial processing of a specimen within a laboratory’s framework. This step ensures the sample was adequately sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any lacking data entry if an electronic chain of custody system did not supply it.

Screening involves a preliminary and rapid check for substance misuse. While it's a cost-efficient method to exclude drug usage in most samples, a positive screen requires confirmation to be legally accepted. Any samples showing presumptive positive results in Screening necessitate secondary confirmation.

Should a sample display a presumptive positive in Screening, additional hair is retrieved from the original sample and readied for Extraction. At this juncture, drugs are extracted from hair at much lower concentrations than other methods (e.g., urine or oral fluid), contributing to the complexity of hair drug testing.

Verification of any positive Screening outcome is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to Confirmation as dictated. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
