What it is, How it works

Hair testing has evolved into a significant method for identifying drug and alcohol consumption. Hair serves as a long-lasting record of alcohol and other substances by capturing biomarkers within the fibers of the growing hair strand. When obtained near the scalp, hair can offer up to approximately a 3-month detection timeframe for alcohol and other drugs. Hair is straightforward to collect, somewhat challenging to adulterate, and convenient to transport.

A 1.5-inch segment of about 200 strands of hair (roughly the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, the optimal quantity for screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is advised. We recommend using a jeweler’s scale to weigh the specimen. If scalp hair is not available, an equivalent quantity of body hair may be used. When referring to head hair, we mean scalp hair exclusively. Body hair refers to all other hair varieties (facial, axillary, etc.).

Process Overview

The key steps in laboratory processing of a drug test result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample entering a lab's system. This involves verifying that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not provided by an electronic chain of custody system.

Screening consists of a preliminary quick scan for drugs of abuse. Although Screening is an economical way to eliminate drug use in the majority of samples, a positive screen must be confirmed to be admissible in court. Samples that show a presumptive positive in Screening require secondary confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methodologies (ex. urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening result is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation as needed. The entire laboratory procedure, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
