If scalp hair is unobtainable, a similar quantity of body hair can be used. When mentioning head hair, it refers exclusively to scalp hair. Body hair encompasses all other hair varieties (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial stage of integrating a sample into a laboratory system. This step includes confirming that the sample was securely sealed and transported correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves an initial rapid assessment for drug abuse. While Screening offers a cost-efficient means to exclude drug use in most samples, a positive result requires confirmation to be valid in legal settings. Samples with a presumptive positive result during Screening necessitate secondary validation.

If a positive result is suggested during the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. In this phase, substances are extracted from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug screening is the most challenging method to undertake.

Confirmation of any positive Screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are washed before confirmation as needed. The complete laboratory process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO/IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
