A 1.5-inch sample of about 200 hair strands (comparable to a #2 pencil) nearest to the scalp provides 100mg of hair, which is the optimal sample size for screening and confirmation. For EtG, additional tests, and/or screens over 10 panels, 150mg of the sample is advised. We recommend using a jeweler’s scale for weighing the sample. Should scalp hair be unavailable, a similar amount of body hair may be retrieved. When discussing head hair, it specifically refers to scalp hair. Body hair comprises all other hair types (facial, axillary, etc.).

Process Overview

The primary stages in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning details the preparatory processing of a sample within a laboratory's system. This process ensures the sample was sealed and delivered accordingly, allocates a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry missing from an electronic chain of custody system.

Screening performs a preliminary examination for substances of abuse. While Screening is a cost-efficient approach to exclude drug consumption in the majority of samples, any positive screen necessitates confirmation to be permissible in court. Samples with presumptive positive results from Screening require a secondary affirmation.

For samples with a presumptive positive result in the Screening phase, more hair is taken from the original sample and prepared for Extraction. At this phase, drugs are isolated from hair at notably lower concentrations than other procedures (e.g., urine or oral fluid), making hair drug screening the most complex methodology to execute.

Confirmation of any positive screening outcomes is conducted using GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation if required. The complete laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
