When cut near the scalp, it offers up to a three-month window for detecting alcohol and drugs. Hair is easy to collect, tough to tamper with, and simple to transport.

A 1.5-inch segment comprising roughly 200 hair strands (approximately the dimension of a #2 pencil) close to the scalp yields 100mg of hair, which is the optimal quantity for screening and confirmation. For EtG, add-on tests, and/or panels exceeding 10, 150mg of hair is advisable. We suggest using a jeweler's scale to weigh the sample. In the absence of scalp hair, an equivalent amount of body hair is acceptable. Head hair specifically denotes scalp hair, while body hair includes all other types (such as facial and axillary).

Process Overview

The lab procedure for drug test results comprises four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of processing a sample within a lab’s framework. It involves ensuring the sample was sealed and dispatched correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not covered by an electronic chain of custody system.

Screening is an initial rapid evaluation for drugs of abuse. Although it’s an economical method to exclude drug usage for most samples, a positive screen must be confirmed for legal admissibility. Samples that preliminarily test positive during Screening need further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the primary specimen and prepared for Extraction. At this juncture, drugs are extracted from hair in considerably lower concentrations compared to other methods (e.g., urine or oral fluid), highlighting the complexity of hair drug testing.

Confirmation of any positive screening outcome is done via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation as necessary. The complete lab process, from Accessioning to Confirmation, is monitored per the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 standards.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
