A 1.5-inch sample consisting of roughly 200 strands of hair (akin to the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the ideal quantity for screening and confirmation. For EtG, add-ons, and/or tests above 10 panels, a 150mg sample is advised. We recommend using a jeweler’s scale for weighing the specimen. If scalp hair isn't available, an equal volume of body hair may be obtained. When referring to head hair, we mean scalp hair only. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial integration of a sample into a lab’s system. This step includes checking that the sample was sealed and transported correctly, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any supplementary data entry not provided by an electronic chain of custody system.

Screening includes a preliminary swift assessment for drugs of abuse. Although Screening is a cost-effective method to exclude drug use in most samples, a positive screen requires confirmation to be admissible in court. Samples that appear positive during Screening need a secondary confirmation.

If a sample appears positive during Screening, additional hair is taken from the initial specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in other methodologies (e.g., urine or oral fluid), which is why hair drug screening is the most complex methodology to perform effectively.

Confirmation of any positive screening result is conducted via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive samples are washed prior to confirmation when appropriate. The entire process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
