For EtG, add-ons, and tests beyond 10 panels, a 150mg specimen is advisable. It's suggested to weigh the specimen using a jeweler's scale. If scalp hair is inaccessible, a comparable amount of body hair can be utilized. "Head hair" specifically refers to hair from the scalp, whereas "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The four pivotal stages in laboratory drug test processing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a lab's database. This step ensures the sample was correctly sealed and sent, assigns a random LAN (Laboratory Accessioning Number), and completes any missing data entry not covered by an electronic chain of custody framework.

Screening involves a preliminary examination for illicit substances. Although Screening is economical for eliminating drug use in most samples, confirmation is necessary for legal validity. Presumptively positive samples from Screening necessitate additional confirmation.

If presumptively positive in Screening, additional hair is extracted from the initial sample for Extraction. In this phase, drug compounds are isolated from hair in much lower concentrations compared to other techniques (e.g., urine or oral fluids), highlighting the complexity of hair drug testing.

GC/MS, GC/MS/MS, or LC/MS/MS are used for Confirming any positive screening findings. Before Confirmation, all presumptively positive samples undergo washing when required. From Accessioning to Confirmation, the entire lab process is evaluated following the CAP (College of American Pathologists) Hair protocol and in compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
