A sample measuring 1.5 inches with around 200 hair strands (comparable to a #2 pencil) closest to the scalp yields 100mg of hair, which is optimal for analysis. For EtG, specialized tests, or assessments exceeding 10 panels, a 150mg sample is advised. It is advisable to use a jeweler’s scale to measure the sample. If head hair is not available, an equivalent amount of body hair can be used. "Head hair" specifically denotes scalp hair, whereas "body hair" includes all other types (such as facial, axillary, etc.).

Process Overview

The drug testing laboratory procedure comprises four principal stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially managed within a laboratory’s system. This step ensures the sample is properly sealed and transported, assigns an arbitrary LAN (Laboratory Accessioning Number), and completes any further data logging absent from an electronic chain of custody system.

Screening conducts an initial rapid evaluation for drug presence. While Screening economically excludes most drug usage cases, a positive result needs verification for legal admissibility. Samples indicating positive results in Screening must undergo further confirmation.

If Screening suggests a positive result, additional hair from the original sample is extracted for further testing. This stage involves extracting drugs from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), emphasizing the complexity of hair drug screening.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS or LC/MS/MS techniques. Prior to confirmation, all samples that are presumptively positive are washed as necessary. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
