Should scalp hair be inaccessible, a proportionate quantity of body hair may be obtained. The term head hair pertains solely to scalp hair, while body hair includes all other types (such as facial or axillary hair).

Process Overview

Four pivotal steps are undertaken in the lab processing of drug test results: Accessioning, Screening, Extraction, and Confirmation.

Accessioning deals with the initial incorporation of a sample into a lab's system. This process involves affirming that the sample was correctly sealed and dispatched, assigning it a random LAN (Laboratory Accessioning Number), and filling in any missing data not included in an electronic chain of custody system.

. Screening conducts an initial rapid assessment for drugs of concern. Though Screening is economical for ruling out drug use in most samples, any positive screen demands further confirmation to hold up in court. Samples that yield a presumptive positive in Screening need secondary confirmation.

If Screening returns a presumptive positive result, additional hair is taken from the initial sample and prepped for Extraction, where drugs are extracted from hair at a much lower concentration compared to other methodologies (e.g., urine or oral fluid), making hair drug screening the most complex methodology to undertake.

Confirmation of any positive screen is carried out through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples require washing prior to confirmation if necessary. The complete lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
