What it is, How it works

Hair can offer a prolonged record of drug and alcohol intake by securing biomarkers within the fibers of the developing hair strand. When collected closely from the scalp, hair can provide a detection range for substances up to roughly three months. This method is easy to collect, harder to tamper with, and simple to transport.

A sample measuring 1.5 inches of approximately 200 hair strands (about the width of a #2 pencil) adjacent to the scalp will yield 100mg of hair, the optimum sample for screening and verification. For EtG supplements and/or tests exceeding 10 panels, a sample of 150mg is recommended. We suggest using a jeweler’s scale to measure the sample. In the absence of scalp hair, a comparable amount of body hair can be collected. References to head hair specifically mean scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial entry of a sample into a laboratory’s system, verifying proper sealing and shipment of the sample, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves a preliminary examination for illicit substances. While Screening offers an economical solution for eliminating drug usage in most samples, any positive screen must undergo confirmation for legal admissibility. Any samples that render a presumptive positive during Screening necessitate further confirmation.

Upon a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are retrieved from hair at much lower concentrations than other methodologies (ex. urine or oral fluid), which makes hair drug analysis a challenging methodology to execute.

Verification of any positive screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples presumed positive undergo a wash before confirmation as required. The entire laboratory sequence from Accessioning to Verification is audited under the CAP (College of American Pathologists) Hair designation and in alignment with ISO / IEC 17025 accreditation norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
