What it is, How it works

Hair testing is now recognized as a powerful means for identifying drug and alcohol consumption. Hair captures a long-term record of alcohol and other substances, as it traps biomarkers within the fibers of growing hair strands. When collected near the scalp, hair can offer an approximate 3-month detection window for alcohol and drugs. Hair is easy to gather, somewhat resistant to tampering, and straightforward to transport.

A 1.5-inch sample comprising around 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp will provide 100mg of hair, which is the optimal sample amount for analysis and confirmation. For EtG, additional screenings, or tests beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If there is no scalp hair available, an equivalent amount of body hair can be used. By head hair, we mean only scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the first step of processing a sample into a lab’s system. It entails verifying if the sample was appropriately sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any extra data entry absent from an electronic chain of custody system.

Screening is an initial rapid evaluation for drugs of abuse. Although Screening is a cost-effective method to eliminate drug usage in most samples, any positive screen must be confirmed to be admissible legally. Samples that presumptively test positive during Screening need a subsequent confirmation.

If a sample shows a presumptive positive in the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. During this step, drugs are extracted from the hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is done via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is conducted under the oversight of both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
