What it is, How it works

Hair can capture a retrospective record of alcohol and other drugs by embedding biomarkers within the fibers of the developing hair strand. If collected near the scalp, hair can offer up to roughly a 3-month detection period for alcohol and drugs. Hair is straightforward to collect, fairly challenging to tamper with, and convenient to transport.

A 1.5-inch segment comprising about 200 hair strands (approximately akin to a #2 pencil) closest to the scalp serves to gather 100mg of hair, which is optimal for screening and confirmation. For EtG, additional tests, and/or tests beyond 10 panels, 150mg of the sample is advised. It is suggested to weigh the sample on a jeweler’s scale. If scalp hair is unavailable, an equivalent quantity of body hair can be procured. When discussing head hair, we mean scalp hair exclusively. Body hair encompasses all other hair categories (such as facial, axillary, etc.).

Process Overview

The key steps in the laboratory processing of a drug test result encompass Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the preliminary processing of a sample into a laboratory’s framework. This involves ensuring that the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry not furnished by an electronic chain of custody system.

Screening comprises an initial rapid assessment for substances of abuse. While Screening serves as an economical way to dispel drug usage in most samples, a positive screen requires confirmation to be valid in legal proceedings. Any samples that are presumptively positive in Screening necessitate a follow-up confirmation.

If a sample is presumptively positive during the Screening phase, more hair is extracted from the original specimen and readied for Extraction. At this stage, drugs are extracted from hair at considerably lower concentrations compared to other methodologies (such as urine or oral fluid), underscoring why hair drug screening is among the most challenging methodologies to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
