What it is, How it works

Hair testing is recognized as an effective method for identifying drug and alcohol consumption. Hair offers an extended record of substance use by encapsulating biomarkers within the strands as they grow. When hair is taken from near the scalp, it can reveal substance use for up to roughly 3 months. Hair samples are easy to collect, challenging to tamper with, and simple to transport.

A sample measuring 1.5 inches, consisting of about 200 hair strands (comparable to a #2 pencil thickness) from the scalp yields 100mg, which is the optimal amount for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, 150mg of the sample is advisable. It's best to measure the sample using a jeweler’s scale. If scalp hair is not available, a similar quantity of body hair may be used. When discussing head hair, we mean scalp hair specifically. Body hair includes all other types (such as facial or axillary).

Process Overview

There are four primary phases in the lab processing of drug test outcomes: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initiating the sample's processing within a lab system. This step ensures the sample was properly sealed and sent, assigns a unique LAN (Laboratory Accessioning Number), and completes any missing data entry that isn’t supplied by an electronic chain of custody system.

Screening provides a preliminary check for substance abuse. Though cost-effective for ruling out drug use in most cases, a positive Screening must be confirmed for it to be legally acceptable. Presumptive positives from Screening require further confirmation.

Upon a presumptive positive result in Screening, additional hair from the initial collection is used for Extraction. In this phase, drugs are extracted from hair at low concentrations compared to other methods (such as urine or oral fluid), making hair drug screening especially challenging.

Confirmation of positive screening results is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive are cleansed before confirmation if necessary. The complete laboratory workflow from Accessioning to Confirmation undergoes evaluation based on both CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standard accreditation.
