Collected near the scalp, hair offers a window of detection for up to about 90 days for alcohol and other substances. It's easy to collect, challenging to tamper with, and straightforward to send.

A sample length of 1.5 inches comprising about 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp equates to 100mg of hair, which is the preferred quantity for both screening and confirmation. In cases of EtG, additional tests, or for panels exceeding 10 items, 150mg is suggested. We advise measuring the specimen on a jeweler's scale. If scalp hair isn't available, a comparable amount of body hair can be used. When referring to head hair, it denotes only scalp hair. Meanwhile, body hair encompasses all other varieties (facial, axillary, etc.).

Process Overview

There are four primary stages in the lab processing of a drug test: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary step of a sample's journey into a lab's framework. It verifies proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any additional data entry that's not included by an electronic chain of custody system.

Screening provides a swift initial inspection for drugs of misuse. While it is a budget-friendly option to rule out drug use in the majority of samples, a positive screen must be substantiated to stand in court. Samples that test positive in Screening necessitate a subsequent confirmation.

If a sample yields a positive result during Screening, additional hair is extracted from the original sample for the Extraction process. In this phase, drugs are retrieved from hair at considerably lower concentrations compared to other techniques (e.g., urine or oral fluid), attributing to the complexity of performing hair drug screening.

Positive screening outcomes undergo Confirmation via GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples are cleaned before confirmation as required. The entire laboratory protocol from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair designation along with the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
