What it is, How it works

When sourced close to the scalp, hair can indicate alcohol and drug usage up to approximately a 3-month period. Collecting hair is straightforward, hard to tamper with, and convenient for shipping.

A sample measuring 1.5 inches and consisting of around 200 hair strands (comparable to the width of a #2 pencil) near the scalp provides 100mg of hair, which is ideal for screening and confirmation purposes. When testing for EtG, add-ons, and/or tests exceeding 10 panels, about 150mg of the sample is advised. Using a jeweler's scale to weigh the specimen is suggested. In circumstances where scalp hair is not accessible, a comparable quantity of body hair can be gathered. When we mention head hair, specifically, we mean scalp hair. Body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into a laboratory's system. This entails ensuring the sample was appropriately sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not covered by an electronic chain of custody system.

Screening executes an initial rapid assessment for drugs of abuse. Though Screening is a cost-efficient method to exclude drug use in the majority of samples, confirmation is necessary for a positive screen to be legally defensible in court. Samples with a presumptive positive in Screening require secondary confirmation.

If Screening suggests a sample is presumptively positive, additional hair is retrieved from the original specimen and prepared for Extraction. At this stage, drugs are extracted from hair at a much lower concentration than other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging technique to execute.

GC/MS, GC/MS/MS, or LC/MS/MS is used to confirm any positive screening result. Any potentially positive samples are washed prior to confirmation if required. The entire lab process, from Accessioning through Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
