For EtG, additional tests, and tests exceeding 10 panels, 150mg of the sample is advisable. We suggest using a jeweler’s scale for measurement. If scalp hair is not available, an equivalent amount of body hair can be collected instead. When we mention head hair, it pertains only to scalp hair. Body hair covers all other hair types (facial, armpit, etc.).

Process Overview

The primary stages in processing a drug test result in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step that integrates a sample into the lab’s workflow. It ensures that the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not handled by an electronic chain of custody system.

Screening provides an initial rapid assessment for illicit substances. Though cost-effective for ruling out drug use in most samples, positive results in Screening must be confirmed to be acceptable in legal proceedings. Samples with presumptive positive results in Screening need subsequent confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the original sample for the Extraction phase. This phase involves isolating drugs from the hair at much lower concentrations compared to methods like urine or oral fluid testing, which is why hair analysis is the most challenging drug testing methodology.

Positive results from Screening undergo Confirmation through GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are washed as necessary before confirmation. The process from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) guidelines for hair and ISO/IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
