Obtaining a 1.5-inch segment of around 200 hair strands (approximately the diameter of a #2 pencil) close to the scalp yields 100mg of hair, which is ideal for both screening and confirming. For EtG, expanded panels, or analysis exceeding 10 panels, it's advised that 150mg of the sample is used. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is not obtainable, an equivalent quantity of body hair can be used instead. When we specify head hair, we mean scalp hair specifically. Body hair encompasses all other types, such as facial or axillary hair.

Process Overview

The drug test result processing within a lab involves four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial admission of a specimen into a laboratory's workflow. This step ensures the sample’s integrity by checking that it was sealed and sent correctly, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any supplemental data entry missing from an electronic chain of custody system.

Screening offers a preliminary, rapid inspection for drugs of abuse. Though economical for excluding drug presence in most specimens, a positive screen requires confirmation to hold up in legal proceedings. Any samples presumed positive during Screening need subsequent confirmation.

For samples potentially positive in the Screening phase, additional hair is extracted from the original sample to prepare for Extraction. Here, substances are withdrawn from hair at lower concentrations than other techniques (like urine or oral fluid), underscoring the complexity of hair drug screening.

Confirming any preliminary positive result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples flagged as presumptive positives undergo washing before confirmation as necessary. The complete laboratory process, from Accessioning to Confirmation, is evaluated under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
