For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is suggested. Use a jeweler's scale to weigh the sample. If scalp hair is unavailable, collect an equivalent amount of body hair. Head hair refers exclusively to scalp hair, while body hair includes all other hair types (facial, underarm, etc.).

Process Overview

The lab processing for a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into the lab's system. This step ensures the sample was sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and completes any data entry not provided electronically by the chain of custody system.

Screening conducts an initial rapid assessment for drug presence. Screening is a cost-efficient approach to detect drug usage in most samples; however, positive results require confirmation for court admissibility. Presumably positive samples in Screening necessitate further confirmation testing.

If Screening suggests a positive result, additional hair from the original sample undergoes preparation for Extraction. During Extraction, drugs are isolated from hair, which is a more challenging concentration than other methods (e.g., urine or saliva), highlighting why hair drug screening is technically demanding.

Confirmation of positive screening outcomes is done through GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive receive a wash pre-confirmation if necessary. The complete lab process from Accessioning to Confirmation undergoes review under CAP (College of American Pathologists) Hair standards and conforms to ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
