A 1.5-inch segment consisting of about 200 hair strands (approximately the diameter of a #2 pencil) secures 100mg, the optimum sample size for analysis and verification. For EtG, additional screenings, and/or exams exceeding 10 panels, it is advisable to use 150mg of the sample. Ensuring accuracy, weigh the sample using a jeweler’s scale. Should scalp hair be unattainable, a comparable amount of body hair is permissible. Note that head hair specifically refers to scalp hair, whereas body hair encompasses all alternative types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial sample processing into the laboratory system. This step ensures the sample's proper sealing and shipping, assigns a random LAN (Laboratory Accessioning Number), and performs any additional data entry absent from an electronic chain of custody system.

Screening provides an initial rapid assessment for drug abuse. While Screening is an economical method to eliminate drug use in most samples, a positive outcome requires confirmation to be legally valid. All presumptive positives from Screening need secondary confirmation.

Should a sample indicate a presumptive positive during Screening, additional hair is derived from the initial sample and set for Extraction. In this stage, drugs are extracted from hair at a lower concentration relative to other methods (e.g., urine or oral fluid), making hair drug screening more complex.

Positive Screening confirmations are performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary washing before confirmation. Every step from Accessioning to Confirmation is conducted under CAP (College of American Pathologists) Hair recognition and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
