For EtG, add-ons, and/or tests exceeding 10 panels, it is advisable to use 150mg of the sample. We recommend using a jeweler’s scale to measure the specimen's weight. If scalp hair is not available, a comparable volume of body hair can be used. When mentioning head hair, it specifically refers to scalp hair only. Body hair encompasses all other types (like facial, axillary, etc.).

Process Overview

The four key stages involved in the laboratory handling of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

The Accessioning step entails the initial integration of a sample into the lab’s system. This process includes verifying that the sample was appropriately sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing any missing data that an electronic chain of custody system hasn’t provided.

The Screening phase includes a preliminary rapid check for drugs of abuse. While effectively ruling out drug usage through Screening is cost-effective for most samples, a positive screen must be confirmed for legal acceptability. All samples that test presumptively positive during Screening need a secondary confirmation.

In cases where a sample shows presumptive positivity at the Screening phase, more hair is extracted from the original sample for the Extraction stage. This phase involves retrieving drugs from the hair at a considerably lower concentration compared to other techniques (such as urine or oral fluid testing), which is why hair drug testing is the most challenging method to execute.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS methods. All samples with presumptive positives are washed before confirmation if necessary. The entire laboratory procedure from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair classification and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
