What it is, How it works

A sample measuring 1.5 inches from about 200 strands of hair (roughly the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal quantity for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, a sample of 150mg is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is not accessible, an equivalent amount of body hair can be collected. Referring to head hair pertains exclusively to scalp hair, while body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The primary steps in the lab processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a sample into the laboratory's system. This process checks that the sample was properly sealed and shipped, assigns a random Laboratory Accessioning Number (LAN), and completes any extra data entry not provided by an electronic chain of custody system.

Screening involves an initial rapid assessment for substances of abuse. Screening is a cost-efficient method to exclude drug use for most samples; however, a positive result requires confirmation to be valid in a legal context. Any samples with presumptive positives in Screening must undergo a secondary confirmation.

If a sample yields a presumptive positive during Screening, additional hair is taken from the original specimen and readied for Extraction. Drugs are extracted from hair at much lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most complex method to execute.

Confirmation of any positive screening outcome is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation if needed. The entire lab process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and accreditation with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
