What it is, How it works

Hair retains a historical record of substance use by embedding biomarkers within the fibers of the developing hair strand. When procured near the scalp, hair offers an approximate three-month window for detecting alcohol and other substances. Collecting hair is straightforward, challenging to tamper with, and convenient to transport.

A sample measuring 1.5 inches with roughly 200 hair strands (comparable to a #2 pencil) from the area closest to the scalp yields 100mg of hair, which is the optimal sample size for analysis and verification. For EtG, additional tests, and/or screenings exceeding 10 panels, 150mg of the sample is advisable. It is suggested to weigh the specimen using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be used. References to head hair specifically denote scalp hair, whereas body hair encompasses all other types, including facial and axillary hair.

Process Overview

The four primary procedures in the laboratory management of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial stages of incorporating a sample into a laboratory’s framework. This process ensures the sample was correctly sealed and dispatched, assigning it a unique LAN (Laboratory Accessioning Number), and performing necessary data entry not facilitated by an electronic chain of custody system.

Screening serves as a preliminary assessment for substances of abuse. Though Screening is a cost-friendly approach to eliminate drug presence in most samples, a positive screen mandates confirmation for court validity. Presumptively positive samples in Screening warrant further affirmation.

Should Screening suggest a positive result, additional hair is taken from the original sample and prepared for Extraction, where substances are isolated from hair at a significantly lower concentration than in other methods (such as urine or oral fluid). This highlights why hair drug screening is a more intricate methodology.

Positive screening results are verified using GC/MS, GC/MS/MS, or LC/MS/MS techniques. Presumptive positive specimens undergo washing before confirmation as needed. The entire laboratory protocol from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the compliance with ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
