A sample of 1.5 inches comprising approximately 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal quantity for testing and validation. For EtG, supplementary add-ons, and/or for tests exceeding 10 panels, 150mg of the sample is suggested. We advise measuring the sample on a jeweler’s scale. If scalp hair is not available, a comparable amount of body hair can be used. When mentioning head hair, we mean scalp hair exclusively. Body hair refers to any other hair types (facial, axillary, etc.).

Process Overview

The four core steps in the laboratory processing of a drug test result comprise Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial handling of a sample into a lab’s system. This step includes checking that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening consists of an initial cursory examination for drugs of abuse. While Screening is a cost-efficient method to dismiss drug usage in the majority of samples, a positive screen should be verified for it to be legally valid. Any samples showing presumptive positive results in Screening necessitate subsequent confirmation.

If a sample shows a presumptive positive in the Screening stage, more hair is extracted from the original sample and readied for Extraction. In this phase, drugs are removed from hair at a significantly lower concentration than other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation as required. The comprehensive laboratory process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
